Valore prognostico dei biomarcatori nel predire la mortalità cardiovascolare in una coorte di anziani istituzionalizzati

BACKGROUND: Heart failure (HF) has been defined a modern pandemic. The complex array of physiologic, psychological, social and health care delivery issues makes it a challenging chronic disease to manage. Thus, our study sought to evaluate a multimarker approach to assess a prognosis of HF elderly patients.METHODS: We considered 202 elderly in the nursing home in Chioggia (Venice, Italy). Each subject underwent clinical evaluation, echocardiography, determination of high sensitivity troponin I (hs-TnI), galectin-3 (GAL), cystatin C (CYS) and brain natriuretic peptides (BNP) using routine methods. These subjects were followed for 24 months after their inclusion in the study and during this period mortality due to cardiovascular diseases (SM) and mortality due to other diseases (OM) were recorded.RESULTS: After a 24-months follow-up (FU), OM was 24% and SM was 18%. For BNP the relative risk (RR) was 2.5 for OM versus 7.5 for SM, for CYS 8.6 vs. 7.5, for GAL 2.7 vs. 5.6, for hs-TnI 2.2 vs. 3.8 respectively.CONCLUSIONS: In the considered population BNP, GAL and hs-TnI, but not CYS, were associated with mortality due to cardiovascular diseases.
