The adjustable periurethral balloons (APUB - ACT™, Uromedica, Plymouth, MN), developed to treat urinary incontinence resulting from intrinsic sphincter deficiency (ISD), might be viewed as a valuable tool in this heterogenous population. In this work we analyzed efficacy and morbidity of APUB in female CSUI. METHODS: Medical charts of women with APUB implantation between January 2010 and December 2019 in our tertiary center was conducted. CSUI was defined as a recurrent SUI after 1 or more surgical procedures, SUI after radiotherapy, neurogenic SUI and other SUI attributable to ISD (Urethral closure pressure < 20cmH20). Efficacy was evaluated by survey with the Patient Global Impression of Improvement (PGI-I) scale involving 7 levels for clinical improvement or worsening (Lickert scale). Success was defined as a social continence with a +2 or +3 PGI-I. The number of APUB adjustments, complications and explantation were collected. This study was approved by the French Research in Urology Ethic Board (CERU 2019/006). RESULTS: Between 2010 and 2019, 53 patients had a APUB for CSUI with a median follow-up of 33 months [1-100]. Twenty patients out of 53 had a neurological disease. Among the rest, 5 had previous radiotherapy and 22 had a neurogenic sphincter deficiency. In 62% (33/53), success was observed at the latest news after 1 to 3 implantations. The median number of adjustments was 3 [1-9]. After the first implantation, the number of prothesis infections, cutaneous or urinary tract exposure and device migration were respectively 3 (5.6%), 9(17%) and 12 (22%), all leading to a APUB explantation. All the complications and explantations were treated in outpatient clinics. In 29 of the initially « success » patients after the first implantation, the median lifetime of APUB was 24 months (1-96). Twelve of them had a new implantation with 11 successes. CONCLUSIONS: APUB can satisfy 62% of female patients with CSUI. These results were obtained with minimally invasive surgery without new urethral surgical access and must be balanced with their significative levels of complications and explantations.
