Insuficiencia mitral en la enfermedad de Barlow. La mirada desde la reparación

La insuficiencia mitral degenerativa es la segunda indicación más frecuente en cirugía cardiovascular. Dentro de ella, la enfermedad de Barlow sigue siendo un desafío a la hora de abordar su reparación quirúrgica. La complejidad y deformidad de la estructura anatómica valvular y los cambios anatomopatológicos en el tejido valvular y en todo el esqueleto fibromuscular, con alteraciones en la arquitectura ventricular, hacen que la reparación sea difícil. Exige un conocimiento preciso de la enfermedad en sus vertientes anatómica, funcional y hemodinámica que nos permita ver más allá de la clínica que presente el paciente para frecerle el mejor tratamiento. En este artículo se hace uma amplia actualización de esta enfermedad, su diagnóstico y su tratamento quirúrgico. Degenerative mitral regurgitation is the second most frequent indication in cardiovascular surgery. Barlow's disease continues to be a challenge for surgical repair. The complexity and deformity of the anatomical valve structure, anatomopathological changes that involve the valve tissue and the fibromuscular skeleton, with alterations in the ventricular architecture make its repair difficult. It requires precise knowledge of the disease and its anatomical, functional, and haemodynamic aspects to allow us to see beyond the clinical presentation of the patient to offer the best treatment. This article provides an update on this disease, its diagnosis, and its surgical treatment.
