•A high rate of patients with oral cavity cancer were found to have an intraoperative positive margin.•Most patients with an intraoperative positive margin were able to undergo further resection to yield a final negative margin.•Regardless of final margin status, patients with intraoperative positive margin were found to have worse outcomes.
