•Sarcopenic patients experience more frequent toxicities prior to radiotherapy.•Sarcopenia is an independent adverse prognostic factor for RT-induced dysphagia.•Besides dysphagia, sarcopenia is not associated with acute RT-induced toxicities.•Sarcopenia does not improve the current NTCP models for acute toxicities.
