•Research on opioid withdrawal lacks a person-centered approach.•Based on Latent Class Analysis, 3 patterns of opioid withdrawal were identified.•Latent classes included mild, moderate, and severe opioid withdrawal.•Nearly 60% of chronic pain patients experienced psychological withdrawal symptoms.•More severe withdrawal classes showed higher rates of problematic opioid use.
