•Male breast cancer is a rare entity.•Ductal Carcinoma In situ (DCIS) is confined within ducts and lobules and rarely metastasizes.•Aggressive phenotype of DCIS may be responsible for metastases rather than diagnostic or treatment variables.•Regular follow up and surveillance is necessary after mastectomy for pure DCIS.
