•Symptoms of acute distress disorder, anxiety, depression, pain, and sleep disturbance are highly prevalent after TOI.•TOI survivors experience multiple, co-occurring symptoms in distinct symptom cluster profiles after injury.•Sociodemographic characteristics and biomarker data are associated with symptom cluster profile membership.•Clinical characteristics are not associated with symptom cluster profile membership.
