Proposal for a New Vesical Imaging-Reporting and Data System (VI-RADS)-Based Algorithm for the Management of Bladder Cancer: A Paradigm Shift From the Current Transurethral Resection of Bladder Tumor (TURBT)-Dependent Practice

•Currently, TURBT is the essential first step in the management of bladder cancer.•TURBT has limitations, including complications, and delay to radical cystectomy.•VI-RADS is a standardized reporting of MRI to accurately predict muscle invasion.•We propose a new VI-RADS-based algorithm for the management of bladder cancer.•It enables avoidance of unnecessary second TURBT and delay to radical cystectomy.
