M1-2 for Buccal Sulcus Mucosal Flap (BSMF)

This patient was operated for right sided unilateral complete cleft palate but developed anterior fistula involving the alveolus and anterior half of the hard palate. Patient complains of nasal regurgitation and defective speech. There is moderately weak intraoral pressure to produce consonants with moderate nasal air emission. As per Pakistan Comprehensive Fistula Classification, this patient has fistula adjacent regions; FAR; M1-2, size is S4, and velum functional status is V1.Plan of Management: – For M1-2, turn-in flaps will be used for nasal layer closure, while oral layer closure will be provided by buccal sulcus mucosal flap.
