P58-3 Practice of oncogene panel testing at regional core hospitals – about the role of a pharmacist

Our hospital was designated as a “Cancer Genome Medical Cooperation Hospital” in collaboration with Keio University Hospital (Core Hospital for Cancer Genome Medicine) in 2018, and as of September 2020, it is the only cooperation hospital in the South Hokkaido area. Pharmacists are required to have cross-disciplinary involvement, such as proposing drug therapies based on genetic profiles. When conducting a genetic panel test, the test will be carried out after receiving sufficient explanation from doctors and nurses and obtaining consent, and the results will be known in about 1 to 2 weeks. After that, MTB (Molecular Tumor Board) will be held at Keio University Hospital. An EP (Expert Panel) will be scheduled after MTB, but before EP, we will hold a CTB (Clinical Tumor Board). In CTB, the result of the panel examination is interpreted using DB etc., and the treatment method for the patient is examined. In EP, based on the CTB results, Actionable / Druggable / Germline genes will be examined, and recommended treatments based on the gene profiling results and recommended treatments based on comprehensive judgment for the patient will be examined. The pharmacist will examine the Actionable / Druggable gene, examine the means of reaching the drug in Japan, and investigate and report on the existence of related clinical trials / clinical trials at CTB / EP. In addition, regarding recommended treatment based on comprehensive judgment for the patient, we propose drug therapy based on the appropriate evidence for the patient, taking into consideration the presence or absence of insurance coverage, place of residence, treatment history, and general condition. We will report on the implementation status of gene panel tests, the breakdown of gene mutations that have reached treatment, the problems with which pharmacists are faced, and future prospects.
