Abstract
Abstract Case: We present the first case of CSF anti-MOG antibody positivity in the setting of acute neurosyphilis. We describe a forty-five-year-old man who presented with acute, unilateral visual disturbance secondary to papillitis and intermediate uveitis. CSF analysis was positive for neurosyphilis and anti-MOG antibodies. Vision improved following antibiotic therapy and serum anti-MOG antibodies were not detectable after recovery. We explore the clinical implications of these findings in relation to antibody testing and monitoring for MOG antibody-associated disorders and possible associations between these disorders and infection. Background: Seropositivity, but not CSF positivity, for anti-MOG antibodies, has previously been described in infectious illness.
