Latin American recommendations for the practice of teledermatology

espanolLas tecnologias de informacion y comunicacion (TIC), incluida la teledermatologia, han cobrado mayor relevancia durante la actual pandemia por COVID-19, y se ha convertido en un instrumento estrategico para la atencion a la salud debido a las restricciones que afectan a la atencion presencial. Material y metodos: Para un mejor uso de la teledermatologia, la Sociedad Mexicana de Dermatologia y el Colegio Ibero-Latinoamericano de Dermatologia (CILAD) emiten las presentes recomendaciones de orientacion. Se determinan tres escenarios para la teledermatologia: el envio de resumenes clinicos, la atencion en tiempo real y la interconsulta a distancia. Resultados: Las consultas de teledermatologia deben ser resultado de una seleccion de pacientes, y efectuarse en plataformas de videocomunicacion privada y recursos para resolver cortes de comunicacion. Debe considerarse la estructura de discurso y presentacion de los participantes en esta modalidad comunicativa, recurrir a tutoriales educativos sobre uso de las TIC y estructurar expedientes digitales. Conclusion: En la videoconsulta se debe instruir al paciente en su forma de presentarse ante la camara, el manejo de esta y el envio de materiales. Se proporcionan las especificaciones para el etiquetado de metadatos, almacenamiento, recuperacion y transmision de imagenes, adecuados a la naturaleza clinica de la comunicacion a distancia. EnglishInformation and Communications Technologies (ICTs), including Teledermatology, have become more relevant during the current Covid-19 pandemic, becoming a strategic instrument for health care due to the restrictions that affect face-to-face care. Material and methods: For better use of Teledermatology, the Sociedad Mexicana de Dermatologia y el Colegio Ibero-Latinoamericano de Dermatologia (CILAD) issue these guidelines. Three scenarios are determined for the performance of Teledermatology: the sending of medical records, real-time consultation, and online referral. Results: Teledermatology consultations must be the result of a selection of patients and have to be carried out on private video communication platforms and resources to resolve remote communication problems. Conclusion: The discourse structure and presentation of the participants in this communication modality must be taken into account, resorting to educational tutorials on the use of ICTs and structuring medical digital records. In the video consultation, the patient should be instructed, in the way they appear before the camera, how to handle it, and how to send materials. Specifications are provided for metadata tagging, storage, retrieval, and transmission of images, adapting them to the clinical nature of remote communication.
