•Postoperative pain management for multiple-level posterior spine fusion surgery remains a major challenge.•Thoracolumbar dorsal ramus nerve (TDRN) block is a novel regional analgesic technique for posterior spine fusion surgery.•TDRN catheters may be a useful adjunct for analgesia in opioid-tolerant patients undergoing spinal fusion surgery.
