•Thromboprophylaxis should be considered in patients with ALK/ROS1+ stage IV lung cancer requiring systemic treatment.•Cautiously reintroduce an effective antiangiogenic agent after thrombosis.•Anticoagulation should be maintained for >6 months in patients with metastatic cancer on active treatment.•If rethrombosis, rule out active cancer, infratherapeutic dose, or anticoagulation failure.•Treat thrombosis with LMWH not requiring dose titration (CrCl>30) or stop (CrCl<15).
