•Patients with OAC failure presented with high residual stroke/TIA risk after LAAC.•With unremarkable bleeding history, bleeding risk after LAAC was low.•Concomitant long-term OAC or prolonged DAPT should strongly be considered to further mitigate stroke risk in this population.
