Despite contemporary advancements, current therapies are inefficient. Recently, experiments utilizing stem cell therapy have shown promising outcomes. In this study, using a rat model of cavernosal nerve injury (CNI), we sought to discover the effect of intracorporal injection (IC) of adipose-derived stem cells (ADSC) on erectile function (EF). METHODS Sprague-Dawly rats were randomized into four groups of rats (n=8/group): 1) INJ-1 group in which the animals underwent laparotomy (Lap) with CNI and IC injection of ADSC at the time of surgery. 2) INJ-2 group in which the rats underwent Lap with CNI and IC injection of ADSC both at the time of surgery and three weeks post CNI. 3) Crush group underwent Lap with CNI, and 4) The Sham control group underwent Lap and immediate closure. Six weeks after CNI, all four groups received EF evaluation via cavernosal nerve stimulation at 2.5V, 5V, and 7.5V and measurement of the intracorporal pressure response. Penile and major pelvic ganglion tissue was harvested for immunohistochemical (IHC) analysis with green fluorescent protein (GFP). RESULTS Rats in the Crush group demonstrated a significant decrease in EF when compared to Sham control, INJ-1, and INJ-2 at 5V (P=0.001, P=0.002, and P=0.027, respectively) and at 7.5V (P<0.001, P=0.002, and P=0.004, respectively), although these relationships were not observed at 2.5V. The EF was not significantly different between the INJ-1 and INJ-2 groups at all voltages. A graphical comparison of the four groups at each voltage can be seen in Figure 1. IHC analysis of penile and MPG tissue showed that the GFP-labeled stem cells were no longer present in MPG and penile tissue in all groups. CONCLUSIONS : The results of this study confirm the potential of ADSC therapy as an effective treatment for erectile dysfunction. Although IC injection of ADSC caused a significant increase of EF in CNI model at 5V and 7.5V, there was no significant added benefit with multiple ADSC treatments.
