Mps 03-07 blood pressure telemonitoring effectiveness in patients with uncontrolled hypertension

Objective: To assess the efficacy of blood pressure telemonitoring (BPTM) in ambulatory patients referred to specialized hypertension excellence center due to uncontrolled hypertension (UC HTN) considering anxiety and depression. Design and Method: RuHealth.net had been chosen to be an on-line platform for BPTM and remote counseling. Than a 3-months program was designed for the follow-up with 2 mandatory clinical visits provided (baseline and end-of-study). We screened 270 ambulant patients referred to the Almazov Centre. 42 (28 males/14 females, 51.6 ± 17 years) patients met the inclusion criteria: UC HTN (“office” BP >140/90 mmHg, on/off therapy) without severe comorbidity requiring frequent clinic visits and with technical capability and motivation for “approach”. They signed informed consent and filled in HADS, SF-36 scores. 30 sex, age and BP- matched patients were taken as a control group. Results: Active group demonstrated a significant decrease in both systolic and diastolic BP levels (-24/-13 mmHg) compared to controls (-9/-7 mmHg). 31 patients (75%) in BPTM group and only 6 patients (20%) in control group had reached target BP values at the end of the study. BP decrease was accompanied with reduction in anxiety/depression levels (-1,8 и -1,3 points of HADS, respectively). Within 3 months each of the 42 patients in BPTM group had at least one remote counselling. Moreover, 23 patients (56%) requested remote counseling during the first week after program initiation with subsequent change of therapy in 14 (60%) cases. Total amount of requests for remote counselling was 168 (approximately 5 per patient). Conclusions: Results of the pilot study demonstrated that BPTM promotes better BP control compared to a standard care approach. It is also feasible for life quality improvement and reduction of anxiety/depression levels and can be implemented in routine clinical practice
