Granulomatous Myositis as a Manifestation of Chronic Graft-Versus-Host Disease: A Case Series and Review of the Literature

•Chronic graft-versus-host disease (cGVHD) is a frequent complication after hematopoietic stem cell transplantation which rarely present as myositis.•Granulomatous myositis (GM) is a rare inflammatory myopathy that is usually reported with sarcoidosis but has also been described in association with cGVHD in a limited number of case reports.•We report 2 cases of GM as a manifestation of cGVHD. Both cases presented with muscle pain and weakness, similarly to what has been described in the preexisting literature. However, one of these cases exhibited localized and not generalized myopathy, a feature that had not been reported previously. Additionally, both cases presented with hypercalcemia, and acute kidney injury which has been reported in sarcoidosis but not GVHD-associated GM.
