•Photic stimulation induced apparent visual symptoms and a high prevalence of photosensitivity in FCMTE.•Aberrant neural activity in primary visual cortex was consistently indicated by multiple MR indices.•Altered Visual-related networks, especially the visual-motor network, enriched the pathophysiological mechanisms in FCMTE.
