•This is the first meta-analysis to investigate the natural history of solely NFPA.•This study included 1057 patients with NFPA followed radiographically.•NFPA classified as macroadenomas were more likely to grow and undergo apoplexy during follow-up compared to microadenomas.•Macroadenomas may need closer follow-up compared to microadenomas.
