•Global incidence and disability cases of vertebral fractures increased from 1990 to 2019.•High-income regions had higher incidence and disability rate of vertebral fractures in 2019.•Regional conflicts resulted in substantial vertebral fractures and related disabilities.•Osteoporosis treatment is important for the prevention of vertebral fractures.•Supportive policies are needed to face the increasing burden of vertebral fractures.
