Objectives: To investigate the associations between the upright serum aldosterone level and the left ventricular(LV) diastolic function according to the pattern of left ventricular hypertrophy (LVH) in essential hypertension. Methods: Upright serum aldosterone level, left ventricular geometry, transmitral E, A, and DT were measured in 60 essential hypertension patietnts and 20 healthy patients. Results: 1) Serum aldosterone level is higher in eccentric LVH group than in concentric LVH group(103.2 +/- 7.2 versus 59.4 +/- 3.1 pg/ml, p = 0.02). 2) Age(r = -0.32) and aldosterone level(r = -0.33) are independently correlated with E. 3) In eccentric LVH group(n = 30), age is correlated with A(r = 0.55, p = 0.002) and aldosterone level is negatively correlated with E(r = -0.52, p = 0.003). 4) In concentric LVH group(n = 20), only age is correlated with A(r = 0.68, p = 0.001). Conclusion: 1) The upright serum aldosterone level is higher in eccentric LVH group than concentric LVH group. 2) It is negatively correlated with early diastolic filling function not in concentric LVH group but in eccentric LVH group.
