•This is the first study to compare the three common procedures to manage NSCLC-related pericardial effusion.•Simple pericardiocentesis group had lower 1-year RFS rate than balloon pericardiotomy or surgical pericardiectomy group.•Surgical pericardiectomy as management demonstrated an improving OS trend.
