CHARM (''Computerized Heart Acute Rejection Monitoring'') is a new, noninvasive method for graft surveillance after heart transplantation (HTX) based on telemetric recording of intramyocardial electrograms. Rejection sensitive markers have been defined which correlate to the results of endomyocardial biopsy (EMB). EMB-proven rejection grade 2 or higher safely can be detected. Successful monitoring of steroid rejection therapy and the early detection of steroid refractory rejection episodes has been reported. The future perspective of this method is the reduction of the number of EMBs and a tailored immunosuppressive therapy.
