Management of Urological Geriatric Patients in Hospital Healthcare

Abstract
Older urologic patients undergoing operative procedures require thorough pre-operative assessments and planning prior to intervention. Failure to properly assess the preoperative needs of these patients can result in increased morbidity and a poor management and complications. It is a prospective, cross sectional study which included 130 consecutive hospitalized, older, urological patients. All patients underwent a geriatric assessment, data about medical history and chronical medication was obtained. Predominant diseases for admission were cancer (36.2%) and complicated nephrolithiasis/ urolithiasis in 22.3% of patients. Only 3 patients (2.3%) had no other associated diseases, and 5. 21.5% patients are taking 2 different types of drugs per day. The median in all study group for chronic medication is 3 (5-2). The cognitive status was within normal ranges, with no malnutrition, no depression, preserved mobility and independence. The study group as pre-frail with Groningen Frailty Scale median of 3 (6-2). A pre-operatory comprehensive geriatric assessment is necessary for determining the operatory risk, the possible post-operatory evolution, the possible development of complications and to determine rate of mortality. The identification frailty and other geriatric syndromes is a must and a recommendation of numerous medical association for a good management of geriatric urological patients in hospital care.
