Medication errors occur frequently in health care. These errors can be harmful and even lead to death. Improvement of patient safety has become a major goal only recently. Therefore, adhesive labels for syringes according to DIVI/ISO standard 26825 were implemented into the emergency rescue system.After surveying members of the emergency medical service, a product was developed which meets the requirements of emergency medical services. Transferring the clinical standard products to the emergency medical service failed because label displays for syringes were too big for use in an ambulance.Adhesive labels are now stored in a compact loose-leaf (DIN A 6) format and a register is used to arrange the leaves in alphabetical order. Adhesive labels are made of polypropylene and are therefore tear-proof. Labels can be inscribed with a ball pen and stick even on wet surfaces. The labels can be stored for 6 years. All members of the emergency medical service were educated in preventing medication errors. The compact loose-leaf format has been in use since March 2011 and has gained acceptance among all members of the emergency medical service.
