•Most mediastinal parathyroids can be accessed transcervically with liberal use of thymectomy.•In experienced hands, Transthoracic Robotic-Assisted Mediastinal Parathyroidectomy (TTRMP) is a safe and effective approach to mediastinal parathyroid glands.•Multidisciplinary approach combining parathyroid and thoracic surgeons to assist with parathyroid identification, biochemical interpretation, and parathyroid reimplantation.
