•This review assessed the current applications of extended reality in back pain.•Despite broader extended reality search, all interventions included were virtual reality based.•Several methodological factors were found to be highly heterogeneous across all studies.•VR tends to improve pain and motion function outcomes.•Methodology framework for the development of virtual reality treatments should be considered.
