•ACGME Emergency Declaration (Stage 3) was associated with disruption of surgical education and learner well-being.•The number of ACGME Stage 3 institutions decreased over time in the face of increased COVID-19 infections.•The frequency of severe disruption of surgical education decreased in Winter 2020.•The frequency of severe disruption on trainee well-being did not improve throughout the study.•The provision of PPE alone was not sufficient to prevent disruption of trainee wellness.
