•Psychiatric comorbidity is associated with lower odds of response to ECT for depression.•This lower odds of response is primarily among younger and less depressed patients.•The ECT response rates were still higher than reported for pharmacotherapies.•Psychiatric comorbidity should not exclude patients from treatment with ECT.
