•Resistance associated mutation to NS5A and NS3 agents are common among HCV directly acting antiviral failing patients.•Glecaprevir/pibrentasvir retained activity in majority of DAA failing patients.•The frequency of NS3, but not NS5A resistance substitutions decreases over time indicating reversal to drug susceptibility.
