Aims & Objectives: The primary objective was to determine the incidence, preventability and consequence of Adverse Events (AE) in the PICU. Secondary objective was comparing prospective observational data-collection with an existing PICU database and a voluntary reporting system. Methods: Prospective observational cohort study in one PICU with one full-time student collecting data, attending registrar handover and ward rounds, to collect AEs in 300 consecutive admissions. Results: 534 individual AEs were identified and occurred in 53.00% of patient admissions (N = 159). There were 39.50 AEs per 100 patient days; 90.07% were preventable; 99.44% of all events caused minor harm. Current recording methods are less accurate than full-time prospective observation.
