We aim to explore short-term oncological prognosis of adrenocortical carcinoma by comparing clinical data between open with laparoscopic adrenalectomy. METHODS We retrospectively analyzed baseline characteristics, perioperative data and postoperative prognosis of 42 patients with adrenocortical carcinoma receiving open adrenalectomy (n=22) and laparoscopic adrenalectomy (n=20) as primary treatment. RESULTS Open adrenalectomy group had larger maximum diameter of tumor, and less benefits in perioperative data than laparoscopic adrenalectomy. Mean disease-free survival of open adrenalectomy was 44.8±35.1 months, longer than 17.5±10.4 months of laparoscopic adrenalectomy. Rate of 2-year disease-free survival in open adrenalectomy group was higher than laparoscopic adrenalectomy group (61.1% vs 21.4%), but rate of 1-year and 3-year disease-free survival showed no significant difference. All patients with laparoscopic adrenalectomy were found local lesions at the first time of recurrence, while 5 of 13 patients with open adrenalectomy did not appear local recurrence. CONCLUSIONS Open adrenalectomy for adrenocortical carcinoma is superior to laparoscopic approach in disease-free survival and rate of 2-year disease-free survival.
