•Patients with sentinel node (SN)–positive melanoma qualify for adjuvant therapy.•No studies have re-evaluated the diagnosis of initially positive SN biopsies.•Review of 322 patients showed that 11.8% were misclassified as SN positive.•At least 4.3% of patients would have falsely qualified for adjuvant therapy.•SN biopsies should be reassessed by an expert melanoma pathologist.
