•Tmax of CSF MMF peak is 2 hours after plasma showing MMF crossing the blood brain barrier with potential direct penetration into the brain.•NfL CSF levels and GFAP show correlation with disease activity in SPMS.•CSF NfL increase is significantly elevated and precedes clinical symptoms by many weeks.•CSF NfL returns to baseline consistent with clinical improvement.
