老年综合评估是老年医学的核心技术,是基层医疗卫生机构开展老年慢性病管理工作的一个重要工具.改进现有评估模式,探索更加适用于基层推广的老年综合评估新模式,有效整合区域医疗资源,对提高社区老年人生活质量、充分发挥基层医疗卫生机构在老年慢性病管理中的守门人作用具有十分重要的意义.
