枸橼酸钠粉剂抗凝管的制备及其应用评价

Abstract
目的:制备并评价枸橼酸钠粉剂抗凝管在纠正乙二胺四乙酸(EDTA)依赖性假性血小板减少症(EDTA-depen-dent pseudothrombocytopenia,EDTA-PTCP)中的应用效果.方法:取109 mmol/L枸橼酸钠三钠溶液至玻璃管经56℃烘干制成各含量的粉剂抗凝管,用仪器法、光学法、手工法和染色法检测238例不同抗凝剂标本的血小板(PLT)结果,分析其与枸橼酸钠水剂、肝素标本抗凝效果的异同.结果:50例正常人12.8 mg含量枸橼酸钠粉剂抗凝标本的PLT接近EDTA-K2标本结果,差异无统计学意义;采血量为200~600 μL时,枸橼酸钠粉剂标本的PLT与EDTA-K2标本结果差异无统计学意义;除白细胞(WBC)外,枸橼酸钠粉剂标本的PLT、红细胞(RBC)、血红蛋白(HGB)与EDTA-K2标本结果相一致(P>0.05);28例EDTA-PTCP患者枸橼酸钠粉剂、枸橼酸钠水剂、肝素抗凝标本的PLT分别为(193.89±50.62)×109/L、(164.86±48.11)×109/L和(108.79±29.33)×109/L,与手工法结果(199.14±47.99)×109/L相比,前者差异无统计学意义(P>0.05),后两者差异有统计学意义(P<0.01);枸橼酸钠粉剂抗凝标本24 h内PLT结果稳定,镜下未见血小板聚集现象.结论:自制枸橼酸钠粉剂抗凝管的最佳抗凝含量为12.8 mg/管,最佳采血量为200~600 μL,在纠正EDTA-PTCP中效果优于枸橼酸钠水剂抗凝管和肝素抗凝管.
