Abstract
目的 探讨呼出气一氧化氮(fractional exhaled nitric oxide,FeNO)和外周血嗜酸性粒细胞百分率(peripheral blood eosinophilia percentage,EOS%)对支气管哮喘诊断和病情评估的价值.方法 因反复咳嗽、喘息、胸闷等症状就诊患者222例,其中139例确诊为支气管哮喘患者为哮喘组,83例非支气管哮喘患者为非哮喘组,2组均检测FeNO、外周血EOS%和第1秒用力呼气容积(forced expiratory volume in one second,FEV1)占预计值百分比(FEV1 pred%),哮喘组患者行哮喘病情测试(asthma control test,ACT)评分,分析哮喘组FeNO、外周血EOS%、FEV1 pred%及ACT间的相关性,绘制ROC曲线判定FeNO和外周血EOS%对支气管哮喘的诊断效能.结果 哮喘组FEV1 pred% [(75.20±1.59)%]低于非哮喘组[(92.59±1.34)%],FeNO[(59.27±3.08) ppb]和EOS%[(6.583±0.432)%]高于非哮喘组[(19.65±2.13) ppb、(2.282±0.192)%](P<0.05);哮喘组FeNO与外周血EOS%呈正相关(r=0.522,P=0.000),与FEV1 pred%无相关性(r=-0.037,P=0.665);FeNO、外周血EOS%与ACT均呈负相关(r=-0.418,P=0.000;r=-0.396,P=0.000);当FeNO诊断支气管哮喘的最佳截断值为29.5 ppb时,AUC为0.896(95% CI:0.853~0.940),FeNO≥29.5 ppb诊断支气管哮喘的敏感度为78.4%,特异度为91.6%;当外周血EOS%诊断支气管哮喘的最佳截断值为3.72%时,AUC为0.783(95%CI:0.722~0.844),EOS%≥3.72%诊断支气管哮喘的敏感度为70.5%,特异度为86.7%.结论 FeNO和外周血EOS%是辅助诊断支气管哮喘的重要指标,外周血EOS%值越高,支气管哮喘患者病情越严重.
