甲状腺TI-RADS超声指标与肿瘤生物学行为的关系

Abstract
目的 探讨甲状腺影像报告及数据系统(TI-RADS)超声指标与肿瘤生物学行为的相关性.方法 将手术病理证实的103例甲状腺癌患者,共计108枚病灶的TI-RADS超声指标与肿瘤侵犯包膜或包膜外周围组织及淋巴结转移进行相关性分析.结果 (1) 108枚病灶中颈部淋巴结转移56枚;淋巴结未转移52枚;癌组织侵犯包膜或侵及包膜外组织38枚;癌组织局限于腺体内70枚.(2)单因素和Logistic回归分析显示甲状腺癌≥2 cm、结节内微钙化灶发生颈部淋巴结转移增多(P＜0.05),以结节内微钙化灶预测更有价值.(3)单因素分析显示病灶≥2 cm对预测甲状腺癌包膜或包膜外侵犯有较高价值(P＜0.05).结论 TI-RADS超声指标中结节大小和微钙化灶与肿瘤生物学行为具有一定的相关性,可根据甲状腺癌的超声表现初步预测其预后.
