剪切波新算法在乳腺实性结节良恶性诊断中的应用

Abstract
目的 应用实时剪切波弹性成像(shear wave elastography,SWE)技术,探讨乳腺结节内部及周边杨氏模量平均值在乳腺结节良恶性鉴别诊断中的价值.方法 应用SWE技术,测量56位患者72个乳腺结节内部及周边杨氏模量平均值(EImean,ERmean),以病理结果为“金标准”,绘制ROC曲线,判断其诊断价值.结果 (1)良性组:EImean为:(37.65±7.99) kPa,ERmean为:(33.29±10.19) kPa,恶性组:EImean为:(50.87±17.19) kPa,ERmean为:(73.26±22.49) kPa.良恶性病灶EImean、ERmean差异有统计学意义.(2)通过绘制ROC曲线,结节内部EImean曲线下面积(AUC)分别为0.74,临界点为:40.15 kPa,结节周边ERmean曲线下面积(AUC)为0.94,临界点为:44.55 kPa.结论 结节周边ERmean为乳腺结节良恶性鉴别提供了一种新的方法,与结节内部EImean联合使用可进一步提高超声对乳腺良恶性鉴别诊断的准确性.
