Abstract
患者男,78岁,主诉“无明显诱因持续胸痛6h,伴头痛、头晕、恶心、胸闷、气短、出汗”.心电图示窦性心律,心率60次/分,V4-6导联ST段压低.实验室检查:红细胞5×1012/L,血红蛋白150 g/L,血小板182×109/L.主动脉CTA:升主动脉、主动脉弓管腔内附壁低密度影,致管腔轻度狭窄(图1A);左冠状动脉回旋支、右冠状动脉、支气管动脉和肺动脉间见吻合、纡曲并扩张的血管网(图1B、1C),并瘘入肺动脉内,瘘口处可见“射血征”(图1D).CT诊断为“冠状动脉-支气管动脉-肺动脉瘘(coronary artery-bronchial artery-pulmonary artery fistula,CBPF)合并主动脉不典型夹层”.
