磁共振弥散加权成像测量食管癌病变长度的应用价值

Abstract
目的:利用磁共振弥散加权成像(MR-DWI)测量食管癌病变长度,探讨其临床应用价值。方法??对38例食管癌患者,应用MR-DWI、内窥镜、食管钡剂透视、CT扫描等不同方法测量病变长度,根据食管癌病理标本收缩比回推至实际病变长度为标准进行比较,分析各检查方法测量出的食管癌病变长度的差异。结果食管癌病变长度测量由短至长依次为MR-DWI(b?=?800?s/mm2)、内窥镜、实体肿瘤、MR-DWI(b?=?600?s/mm2)、食管钡餐透视、CT扫描检查和MR-DWI(b?=?400?s/mm2),其中MR-DWI(b?=?600?s/mm2)测量的长度与实体肿瘤长度最为接近,且有高度的相关性(r?=?0.980,P?0.05),这几种测量方法所测量的病变长度与实际病变比较,差异有统计学意义(P?0.05)。结论 MR-DWI(b?=?600?s/mm2)扫描可作为食管癌病变长度测量的重要补充手段,与内窥镜、钡餐透视等多种方法结合,可提高术前评估的准确性。
