Abstract
Objective To study the role of brief cognitive impairment rating scale (Cog-12) in assessing cognitive impairments (CI) in elderly chronic obstructive pulmonary disease (COPD) patients.Methods One hundred and seven elderly patients admitted to hospital from February 2013 to November 2015 were divided into COPD group (n=59) and COPD-free group (n=48).They were scored according to the Cog-12,MoCA and MMSE.The number of detected CI was judged by calculating the area under the ROC curve.Results The risk of cognitive impairment in COPD group (74.6%) was higher than that in COPD-free group (43.8%).The incidence of mild CI in COPD group and COPD-free group was 30.5% and 25.0% respectively,and that of AD was 44.1% and 18.8%.Patients with COPD had a significant cognitive impairment in each test compared to COPD-free group.The Cog-12 score was significantly lower while the MoCA and MMSE scores were significantly higher in COPD group than in COPD-free group (15.44±12.60 vs 7.54± 8.09,19.19±7.53 vs 22.91±6.33,22.46±8.38 vs 25.71±6.33,P<0.05).The area under the ROC curve for MoCA,MMSE and Cog-12 was 0.964,0.911 and 0.884 respectvely in COPD group.The sensitivity and specificity of Cog-12 were 82.4% and 78.6% in diagnosis of CI when its cut-off score was 6.Conclusion Cog-12 is a concise and comprehensive tool to screen cognitive function and can thus effectively detect CI in COPD patients.
