胰十二指肠切除术后胃瘫综合征42例临床分析

Abstract
胰十二指肠切除术(pancreaticoduodenectomy,PD)是治疗胰头以及壶腹周围疾病的经典术式之一,手术创伤大,术后并发症发生率高,其中胃瘫综合征(postsurgical gastroparesis syndrome,PGS)发生率位居众多并发症之首.本研究回顾性分析PD术后患者PGS的发生情况,现报道如下.
一、资料与方法
1.一般资料:收集1998年8月至2012年12月实施PD手术的504例患者资料,术后发生PGS 42例.其中男性25例,女性17例,年龄36 ～ 79岁,中位年龄58岁.从发现黄疸至入院时间为4～ 32d,术前总胆红素87 ～ 513 μmol/L,＞ 300 μmol/L者9例.合并糖尿病5例,高血压、冠心病3例,抑郁型精神分裂症1例.病理诊断:胰头癌22例,壶腹周围癌13例,胆管远端癌4例,胰头部慢性炎症1例,胰头部囊实性假乳头状瘤1例,胰腺外伤1例.
