缺血性脑卒中发生发展与高同型半胱氨酸血症的关系探究

Abstract
目的:探讨缺血性脑卒中(cerebral arterial thrombosis,CAT)发生发展与高同型半胱氨酸(homocysteine,Hcy)血症的关系.方法:选取2016-09~2018-09我院收治的CAT患者126例为CAT组,同期选取健康体检者100例为对照组,采用循环酶法检测所有受试者血清Hcy水平.CAT组根据Hcy水平分为2个亚组:高Hcy血症组(Hcy>15 μmol/L)和正常Hcy组(Hcy≤15μmol/L).比较两组内皮型NO合成酶(endothelial nitric oxide synthase,eNOS)、一氧化氮(nitric oxide,NO)、反应性充血后血管内径变化率(flow-mediated diameter,FMD)、颈动脉内膜中层厚度(intima-media thickness,IMT)、美国国立卫生研究院卒中量表(national institutes of health stroke scale,NIHSS)评分及Barthel指数(Barthel index,BI)评分.结果:CAT组血清Hcy水平显著高于对照组,且血清Hcy水平>15 μmol/L患者比例显著高于对照组,差异具有显著统计学意义(P<0.01).高Hcy血症组eNOS、NO、FMD水平及BI评分显著低于正常Hcy组,IMT值和NIHSS评分显著高于正常Hcy组,差异具有显著统计学意义(P<0.01).结论:高Hcy血症作为CAT的危险因素,可反映患者的病情程度及预后,可能与患者血管内皮功能降低相关.
