β-溶血性链球菌红霉素耐药特征与大环内酯类耐药基因的分析

Abstract
目的 了解安徽地区β-溶血性链球菌大环内酯类耐药基因的携带情况,并与红霉素药敏结果进行对比分析.方法 对来源于安徽地区2005-2013年临床分离的61株β-溶血性链球菌采用琼脂稀释法测定红霉素的最小抑菌浓度(MIC),同时应用PCR扩增大环内酯类相关耐药基因.结果 61株β-溶血性链球菌中A群(Group A Streptococcus,GAS)占21.3%,B群(Group BStreptococcus,GBS)占68.8%,C和G群(Group C Streptococcus,GCS、Group GStreptococcus,GGS)共占9.9%,A群及B群对红霉素的耐药率较高,分别为76.9%和85.7%,携带的耐药基因均以ermC基因(76.9%和66.7%)占主导.1株GCS对红霉素耐药且只携带ermB基因,GGS对红霉素的耐药率为60%,以ermB和ermC基因的检出率最高,均为60%.结论 安徽地区β-溶血性链球菌的红霉素耐药率较高,且主要由erm基因介导的重度耐药常见.临床医生应加强耐药监测,依据药敏结果指导用药.
