磁共振水成像技术在动态磁共振子宫输卵管造影的应用

Abstract
目的:探讨磁共振水成像技术(magnetic resonance hydrograph,MRH)在动态磁共振子宫输卵管造影(dynamic MR hysterosalpingography,dMR-HSG)中临床价值.方法:对17例不孕女性患者先后行dMR-HSG和传统X线子宫输卵管造影术(hysterosalpingography, HSG)检查.常规MRI扫描包括横轴位T1WI,横轴位和冠状位脂肪抑制T2WI;在MR磁体内通过导管向宫腔内注射0.9%生理盐水后,利用单次激发厚层投射(single shot fast spin-echo,SS-FSE)水成像技术进行动态子宫输卵管造影.结果:所有患者均能顺利完成两项检查,dMR-HSG能观察盆腔结构、子宫腔、黏膜、肌层及卵巢结构;dMR -HSG表现为双侧输卵管阻塞者4例,输卵管单侧阻塞者5例,子宫、输卵管正常者8例.传统X线检查示左侧输卵管不通,dMR-HSG提示两侧输卵管通畅者1例,其余病例dMR -HSG和传统X线HSG两侧输卵管通畅性诊断结果基本一致.结论:dMR-HSG图像结合常规MRI不仅能显示输卵管结构,还能显示盆腔病变,有助于不孕症的病因诊断.
