体外循环心脏直视手术时的血压及大脑中动脉血流速度的观察

Abstract
目的:探讨体外循环下心血管手术中血压及脑血流的变化.方法:应用经颅多谱勒仪(TCD)测定7例心脏直视下手术患者手术过程中不同阶段的大脑中动脉的血流速度,同时记录其平均动脉压.结果:平均动脉压在麻醉前、后、并行阶段、体外循环(心脏停跳)及心脏复跳后的值分别为:97.66±14.46,94.78±15.72,75.35±20.87,62.07±8.33,85.57±11.23(mmHg);大脑中动脉的平均血流速度分别为:52.20±20.20,35.71±12.29,48.57±22.47,43.71±9.81,65.71±19.78(cm/s).7例患者中4例术后较术前血流速度明显改善 .结论:(1)随着心脏功能改善,大脑中动脉血流速度比较术前亦有改善.(2)通过TCD监测可了解术中脑血流的变化,便于采取措施以避免脑缺血, 对减少和预防体外循环后所致的神经精神损害提供了客观依据.
