病人,女,69岁,以"反复气促13年,双下肢水肿 3年,再发2周"入院.13年前病人出现劳累后气促、胸闷、心悸等症状,4年前外院心脏超声提示风湿性心脏病二尖瓣病变,并行二尖瓣人工生物瓣膜置换术.入院后查体提示心房颤动心律,二尖瓣听诊区可闻及4～6级收缩期粗糙吹风样病理性杂音.气促、下肢水肿等心力衰竭症状明显,不能平卧.
